Assisted Living, in one paragraph.

Assisted Living at Portola Gardens is daily care for adults who need help with the activities of daily life but do not need continuous skilled nursing. Medications are managed by licensed staff. Meals are served three times a day in the dining room. A caregiver is always on the floor. Residents have private or shared suites with their own bathrooms, and the rest of the building is theirs to use.

Is Portola the right fit?

We care for a broad range of residents, ambulatory and non-ambulatory. There are also needs that belong in a different setting, and we will tell you plainly. A quick call sorts it out either way.

A good fit

  • Needs help with medications, meals, bathing, dressing, or transferring
  • Walks independently, or uses a wheelchair or walker
  • Lives with mild cognitive impairment, or dementia without combative behavior
  • Has stable diabetes managed with oral medication
  • Has fallen recently, or is increasingly isolated at home
  • Has a primary caregiver who is burned out
  • Is on hospice (we accept hospice referrals)

Better in another setting

  • Needs rehabilitation or around-the-clock complex medical care
  • Has a pressure wound above Stage 2
  • Needs sliding-scale insulin, or diabetes care several times a day
  • Needs skilled nursing more than a few times a day
  • Has combative or aggressive behavior
  • Needs a licensed memory care unit

Who is Assisted Living for?

Adults who need support with daily activities (medications, meals, bathing, transferring) but do not require continuous skilled nursing. We welcome residents who are ambulatory or non-ambulatory, including those with mild cognitive impairment or dementia, as long as behavior is not combative and care needs don't require frequent skilled nursing.

What is included in monthly pricing?

Suite, three meals daily plus snacks, weekly housekeeping, laundry, utilities, activities, 24-hour staffing, and care services per your care plan. Personal items, salon services, off-site outings beyond standard, and private duty are extra.

What if my care needs change?

Care plans are reviewed every 90 days or sooner on request. If needs increase, we adjust the care plan or coordinate transition to a higher level of care.

Can I tour before committing?

Yes. Tours last about an hour, no charge, no deposit required. We meet you at the door and walk you through the suites currently available.

Do you accept Medi-Cal?

Yes, through the Assisted Living Waiver (ALW) for eligible residents. We are one of three SF facilities in the ALW program. See our ALW guide for eligibility.

What is your staff-to-resident ratio?

Ratios vary by shift and care level. We share current numbers during the tour conversation rather than publishing them online, because the meaningful version requires context about how they map to care plans.
