Portola Gardens is one of three San Francisco County facilities in the California Assisted Living Waiver (ALW) program. We coordinate placements directly with CCAs, IHSS workers, hospital discharge teams, and ADRC.
Placements
Every admission is reviewed individually. The list below is what we see most often, not a hard limit. When in doubt, call admissions and we’ll talk through your client’s situation.

Acceptance into Portola Gardens is determined on a case-by-case basis by our admissions team in accordance with California Department of Social Services RCFE regulations (Title 22) and the resident’s individual assessment. Referral flow
We aim to confirm bed availability and a preliminary care-needs match within one business day.
You send the intake packet (we accept your standard format). We review within 24 hours.
Client and family tour the community, or we host a video tour for out-of-area placements.
We handle CCA paperwork, transportation logistics, and care-plan handoff with your team.
Move-in as fast as 2–3 days. When the cost-of-care breakdown is confirmed, we have placed residents within a few days, working with partners like Star Nursing and the San Francisco Department of Public Health.
