Portola Gardens is one of three San Francisco County facilities in the California Assisted Living Waiver (ALW) program. We coordinate placements directly with CCAs, IHSS workers, hospital discharge teams, and ADRC.

Every admission is reviewed individually. The list below is what we see most often, not a hard limit. When in doubt, call admissions and we’ll talk through your client’s situation.

  • ALW-eligible residents with active CCA
  • Private-pay residents needing assisted living
  • Residents stepping down from a Skilled Nursing Facility stay

These take a conversation, not a checklist.

  • Continuous skilled-nursing or sub-acute medical needs
  • Active communicable conditions or unmanaged acute psychiatric needs
  • Equipment-dependent care (ventilator, dialysis, long-course IV)
  • Bariatric residents above standard equipment capacity

When in doubt, call admissions. Many edge cases are workable with the right care plan.

Acceptance into Portola Gardens is determined on a case-by-case basis by our admissions team in accordance with California Department of Social Services RCFE regulations (Title 22) and the resident’s individual assessment. From your first call to a confirmed bed.

    Initial call or email to admissions

    We aim to confirm bed availability and a preliminary care-needs match within one business day.

  2. 2

    Client packet review

    You send the intake packet (we accept your standard format). We review within 24 hours.

  3. 3

    Tour or virtual walkthrough

    Client and family tour the community, or we host a video tour for out-of-area placements.

  4. 4

    Move-in coordination

    We handle CCA paperwork, transportation logistics, and care-plan handoff with your team.

Move-in as fast as 2–3 days. When the cost-of-care breakdown is confirmed, we have placed residents within a few days, working with partners like Star Nursing and the San Francisco Department of Public Health.
